Sample Orientation Checklist
Below is a sample of a new employee orientation checklist. Feel free to copy the format and adjust to fit your needs. The line before each number is to be initialed or dated as each step is completed. This is your documented proof that the employee has been fully informed of all work rules and procedures, should a dispute occur after termination. Make sure the completed form is kept in employee's personnel file. This document will remind you of items to get back from your employee or passwords to change upon employee's termination.

_____4. Give associate a copy of your Employee Handbook. Ask the associate to read it. Review the Employee Handbook with the associate in about a week, answer questions, and have him or her sign the "have read" section of this document. Retain this document in the employee's personnel file.

_____5. Give associate appropriate job description and discuss it point by point, including standards of performance and behavior. Answer any questions.
_____6. Tell associate how, when, and in what areas he or she will be assigned tasks and will receive training.
_____7. Cover any immediate work rules he or she needs to know.
_____8. Assign the associate a "buddy" for the first week with whom he or she can eat lunch, get directions, and ask questions.
_____9. Make a list of vendors, clients, or other outside people he or she needs to meet and set dates.
_____10. Assure the associate of your pleasure in having him or her be a part of the team and of your confidence in his or her future success.
